Job Summary
Cleveland Clinic London are looking to recruit a Specialist Case Manager to join our Case Management Team, with a competitive salary range on offer, up to £52,000.

What are we looking for
Cleveland Clinic London are looking to recruit a Specialist Case Manager to join our Case Management Team.
As a Specialist Case Manager you will be responsible for assessments, continued review and coordination of proactive comprehensive discharge planning activities and utilisation review collaborating with internal and external stakeholders.
You will work closely with various healthcare professionals to assess, plan, implement and evaluate care options and services that meet the patients’ health needs within Neuro Rehabilitation; while working closely with third party payers to keep them appraised of their patients’ condition and needs; obtaining authorization for as required.
Role information
This full time, permanent position would require the postholder to work 37.5 hours per week.
This is a Monday-Friday, 9am-5pm role.
The maximum salary on offer for this opportunity would be £52,000 and would be dependent upon experience and competencies against the role requirements.
What will your duties include?
  • Perform utilization reviews for all Neuro Rehabilitation patients.
  • Resource/liaise between patient, hospital, physicians and third party payers to optimize patient’s treatment and ensure adequate funding.
  • Participate in multi-disciplinary team meetings to coordinate patient’s care plan and proactive comprehensive discharge planning.
  • Demonstrate behaviour supportive of team achieving department goals.
  • Track and trend denial and appeals to identify issues and develop action plans for resolution.
What we need from you?
  • RGN (Registered General Nurse) with substantial post graduate experience.
  • Neuro Rehabilitation experience.
  • Recent substantial clinical ward experience and or substantial Case Management experience.
  • Experience of utilisation review.
  • Experience of managing multiple stakeholders across a multitude of seniority.
